LIFT is a home based program to help families intervene with their very young children with special needs to get the assistance necessary to promote optimum growth and development. Through the individualized activities of early intervention, many LIFT children will improve in or catch up in key areas of development and will thereby require fewer or no specialized services later in life. Services are provided to families of babies who have developmental disabilities, or are experiencing a developmental delay or are at risk of delay from birth to 3 years old. Babies who do have a developmental disability are eligible to receive transitional case management until the age of 6 years. Staff provides a wide range of support and assistance in the home. Based on the specific needs and concerns of the family, LIFT offers developmental assessment, speech and language assessment, physical and occupational therapy, infant education, service coordination, parent training, parent-to-parent support, community information and referral.
Case Management
To assist children/adults and their families to receive the supports, services, information and assistance necessary to successfully live and function in their natural community as well as providing the administrative link to allied service systems. Upon referral, a case manager is assigned to assist the individual consumer in the development of an individualized service plan to assess and identify areas of need, interest, concern and intervention. The case manager is charged with the responsibility of orchestrating the implementation of the plan and ensuring the individual's needs including residential, financial, vocational, employment, medical, psychological, social and recreational are incorporated and well integrated. Case managers provide ongoing program oversight as well as long-range service planning.
Community Participation Program
Offers intensive daytime support to enable adults with mental retardation access to integrated community life in areas of recreation, volunteerism, and group employment. Community and center-based training activities and/or employment are developed consistent with individual personal interests and attributes. Support and training is provided to enhance self-care, communication, functional education, vocational, social and interpersonal skills, as necessary. Group outings and activities are also provided.

Greenville Avenue Residence
Provides housing and community assistance to adults with mental retardation who desire to live and learn in a congregate environment. Training is provided for seven adults who receive assistance in a variety of general activities of daily living designed to help them become more self-reliant. Training and assistance is provided in such areas as personal care, community participation, recreation, money management, home care, cooking, shopping, medication administration, behavior management and any other areas needed or requested by the residents. Individual program plans are developed to maximize each resident's potential and individuality and minimize the problems often associated with their disability. Residents are supported in developing the ability to learn to live together and to function within the parameters of a typical home with family style routines. Some residents may use this as a stepping stone to a more independent living arrangement.
Grandview Residence ICF/MR
Grandview Residence is a 6-bed residence serving adults with mental retardation and physical/medical disabilities. Grandview Residence provides housing and intensive assistance designed to reinforce the highest level of independence possible. Residents receive comprehensive services to meet their needs in a variety of areas ranging from medication assistance and behavioral support to home care and community guidance. The residents of Grandview have widely diverse needs, interests and abilities. Individualized programs are developed for each person addressing their interests and needs. Residents must meet the criteria for ICF/MR (intermediate care facility for individuals with mental retardation) level of care that typically refers to more severe levels of disability. Residents receive interdisciplinary services including: nursing, psychiatric, psychological, social services, various therapies, pharmaceutical and nutritional services.
Day Support Services are provided at the Orchard Lane site Monday through Friday with the goal of enhancing community involvement and independence

Greenstone Residence ICF/MR
Greenstone is a 14-bed residence serving adults with mental retardation and physical/medical disabilities. Greenstone provides housing and intensive assistance designed to reinforce the highest level of independence possible. Individual residents must meet and maintain HUD eligibility standards. Residents receive comprehensive services to meet their needs in a variety of areas ranging from medication assistance and behavioral support to home care and community guidance. The residents of Greenstone have widely diverse needs, interests and abilities. Individualized programs are developed for each person addressing their interests and needs. Residents must meet the criteria for ICF/MR (intermediate care facility for individuals with mental retardation) level of care that typically refers to more severe levels of disability. Residents receive interdisciplinary services including: nursing, psychiatric, psychological, social services, various therapies, pharmaceutical and nutritional services.
